The Intervention Request Form should be completed by Ministries, Departments and Agencies (MDAs) requesting a Results-Based Management (RBM) intervention or support from the Performance Management & Evaluation Branch (PMEB) in the form of: (1) Informative &/ Instructional Presentations, (2) Activity-Based Workshops, or (3) Capacity Building Training.

The completed Request Form, accompanied by a cover letter signed by the Head of Entity, must be submitted to the Cabinet Secretary.
